preparing for your first visit
We look forward to meeting you on your first visit to our office and have tried to make it easy by offering our patient forms online for a stress free first visit. During this time, we will help you get acquainted with our office and learn about your orthodontic or dental treatment. We understand that each patient’s treatment is different, and we promise to give you the personal attention you deserve. Your first visit will consist of an examination and discussion of potential treatment options. At Wake Orthodontics and Pediatric Dentistry, our team has the skills and the experience to provide all our patient with both orthodontic and pediatric dental care. We utilize a team approach that will last from your child’s first visit at age 1 until they are ready to leave your home and begin their adult lives. We are proud to be one of the only bi-specialty practices in the state of North Carolina specifically catering to children!
Patient Forms
Save time at your already scheduled appointment by filling out our new patient forms prior to your visit! If you do not have a pediatric or orthodontic appointment yet, please do not complete these forms and instead request your appointment first here.
diagnostic records and consultation
Diagnostic records include digital photographs, X-rays, and models of the teeth and jaws. These items help the dentists arrive at a diagnostic and customized treatment plan for the patient.
After the dentists have studied the diagnostic records, a consultation appointment may be needed to discuss the proposed treatment with you. While they are certainly welcome, it is not necessary that young patients attend the consultation appointment.
The fee for the diagnostic records and consultation appointment is $275.00. lf available, current X-rays need not be repeated and may be subtracted from the records fee accordingly.
After the consultation, appointments may vary on an individual basis depending on the treatment to be performed.
